I took the beginners class in high school, and continued afterwards listening to SEVERAL books on tape (or mp3s) where you repeat every sentence and everyone looks at you looks at you like you're crazy, lol, j/k.

Also rented many many Italian movies w/ english subtitles and keep practicing! You really have to be consistent on your own with studying to keep all the rules and language fresh in your memory.

Music helped as well! Sing along with Bocelli, Lucio Battisti, Laura Pausini.

The way I learned to speak Italian fluently was studying in Italy, listening to music, making Italian friends, and falling in love with an Italian.
Honestly... having an Italian boyfriend that does not speak english, has truly helped my language go from conversational to fluent... fluent enough to argue my point... which is important!! Carry a dictionary... listening to music is also helpful..some music I like is : Negramaro, Sugar free, subsonica, and tiromancino. Good Luck!
Also check out craigslist.org and find someone in your area that offers lessons or conversation time. Maybe you can trade off english lessons for italian ones.
